Criminal Background Check
It has become necessary to have criminal background checks performed on all currently enrolled students in Health Sciences programs. This is necessary because of our affiliations with numerous clinical sites.
All applicants accepted into a Health Sciences program will have a criminal background check performed. This will be done through a contracted agency. At the present time, if the program you are planning to enter requires licensure or certification by a national or state organization, it will be the decision of that agency as to your qualifications to apply for licensure or certification. Your acceptance into an OTC Health Sciences program or the completion of the program does not guarantee the eligibility to apply for licensure or certification.
Students with a felony or a significant criminal history may not be able to participate in clinical experiences at some of our sites and may not be able to apply for a license to practice.
Immunization Requirements
If accepted to your program of interest you will be requested to submit documentation for required immunizations, TB testing and current Basic Life Support for Healthcare Providers CPR.

Associate of Science in Nursing Essential Functions
HESI Entrance
Testing Dates for the April 15 deadline (August start). (If you are a paramedic, you do not need to sign up for the HESI Admission Test as you will take this test at the end of your ASN 205 course.) Please review the available slots for testing on the Sign-Up Genius and click on the button to sign up. Once you have signed up, you will receive an email with further instructions regarding paying for the test, study resources and the Microsoft Teams link to join on testing day. YOU MUST BE SIGNED UP AND YOUR TEST MUST BE PAID FOR BY THE THURSDAY BEFORE YOUR TESTING DATE. Your name will be removed from the sign up otherwise.
If you need to change your testing date for another, please delete yourself from the date you do not wish to test. You will then need to send an email to [email protected] to get the Teams link for the new testing date you have selected. If you decide not to test, please delete your sign up so slots are available for others who still need to test.
Applicant must be an LPN with a current unencumbered, undisciplined (Missouri or Compact State) LPN license OR paramedic with a current unencumbered, undisciplined Missouri state or national license.

PROGRAM INFORMATION:
The Associate of Science in Nursing (ASN) Program and the Paramedic to Associate of Science in Nursing Program are hybrid programs with a selective admission process determined by a points-based system. Review the admission scoring guide here.

In the event of a tie in admission scores, verified years of experience as an LPN or paramedic will be the deciding factor.
Ozarks Technical Community College Practical Nursing graduates accepted into the ASN program must take the NCLEX-PN to obtain a current Missouri LPN license within 90 days of PN graduation to remain in the ASN program.
ENGLISH PROFICIENCY REQUIREMENT:
Students whose primary language is not English must document English language proficiency. This requires:
- Minimum TOEFL iBT passing score of 83, with a minimum speaking score of 26.
- Submit a current TOEFL iBT score (Test of English as a Foreign Language) by the application deadline during the online application process.
- Students should register for testing using the TOEFL website. Only tests administered in a professional test center are accepted.
- The TOEFL report scores are valid for 5 years from the test date.
NOTIFICATION OF ACCEPTANCE:
- Based upon the program’s admission scoring system, the top 24 applicants will be offered a position in the program and will receive instructions on the next steps to complete program admission.
- Students will be notified of acceptance or non-acceptance via the online application portal.
- Accepted students will receive an acceptance packet task list through their OTC email account to complete the admission process.

HESI ADMISSION ASSESSMENT EXAM:
- A minimum overall score of 75% must be achieved on the HESI Admission Assessment Exam to be considered for the program.
- This test will be delivered seated or online.
- Student will have 4 hours to complete testing.
- HESI Exam instructions will be provided during the online application process when students START their application for the Associate of Science in Nursing – Direct Entry program. - February 1 – May 1
- July 1 – December 1
- Please note, you MUST take the exam in time to have test results ready by the deadline of the program you are applying to.
- You are allowed a TOTAL of 2 attempts per testing window.
- The cost is $75.00 per test online, and $45.00 per test seated.

- This is separate from the test taken to be admitted to OTC.
SELECTION FOR INTERVIEW PROCESS:
The RT Program is a selective admission program. A grade of “C” or better is required in the pre-admission courses to be considered for the interview process. The Preferred Course Sequence outlines the pre-admission courses.
- A score is given for the grade made in all the preadmission requirements.
- All preadmission courses must be completed by the end of the semester in which the applicant is applying.
- Mid-term grades will be checked in the spring semester the student is applying.
- A GPA of 2.5 and higher is required to receive a score.
INTERVIEW PROCESS:
- Applicants who have met all the requirements are considered for an interview.
- An email regarding the date, time, and location of the Health Sciences interview will be sent approximately two weeks in advance. Please notify Health Sciences of any change in name, email address, or phone number.
- The interview is approximately 15 minutes in length.
NOTIFICATION OF ACCEPTANCE:
- Based upon the program’s admission scoring system, the 20 applicants with the highest score will be offered a position in the program and will receive information on the necessary steps to complete program admissions.

The Computed Tomography Program is a selective admission program, and a points-based system determines admission. To see how points are determined, refer to the Candidate Selection Form. A 2.75 GPA is required to be eligible for this program. Incomplete files will not be considered. Complete the following by the application deadline:
- Complete the Ozarks Tech Admissions process.
- Official transcripts from accredited schools attended other than Ozarks Tech must be received by Ozarks Tech Admissions before the application deadline (preferably 4-6 weeks prior, to allow time for review).
- Submit a CT (Computed Tomography) program application using the online application portal apply.otc.edu by the application deadline (preferably 4 weeks before the deadline).
- Will need to upload your current proof of at least one of the following:

- Provide the institution that provided you with your imaging degree or certificate.

GENERAL EDUCATION CLASSES & THE SELECTION PROCESS
- Points are given for the grades earned in the general education courses outlined on the Candidate Selection Form.
- Complete as many general education courses as possible with the highest grade possible for the best score.
- If applying for the June 15 deadline, general education courses must be completed by the end of the spring semester.
- If applying for the October 31 deadline, general education courses must be completed by the end of the fall A block.
- OTC general education courses will be applied first to the Dental Assisting AAS degree “elective” course requirements. All other transfer courses will be reviewed for applicability on an individual basis.

The Dental Hygiene Program is a selective admission program and admission is determined by a points-based system. Points are awarded based on grades earned in preadmission courses; completion of clinical observation hours, or CDA, or CDA experience equivalent to 3 years; preadmission testing and cumulative GPA.
Complete the following by the application due date. Note: An incomplete file will not be considered.
- Apply to OTC and complete admission processes.
- Complete the prerequisite courses by the application deadline. Student must email [email protected] if:
- a passing grade of “C” or better is not achieved after two attempts in any program prerequisite course requirement, OR
- your Human Physiology course will be more than 5 years old at the time of your application deadline.
- Request official transcripts from accredited schools attended other than OTC. These must be received by OTC Admissions before the application deadline.
- Complete required testing by the application deadline. Registration is required through Testing Services.
- If applicable, upload current Certified Dental Assistant (CDA) documentation by the application deadline.

PHLEBOTOMY REQUIREMENT:
Phlebotomy is a prerequisite for the MLT program. PLB courses are open to non-MLT students if seats are available. The Phlebotomy course may be taken in two ways:
- Option 1: PLB 100 Introduction to Phlebotomy (Spring semester, A block)
- PLB 101 Phlebotomy Clinical (Spring semester, B block)*
*Students must have adequate time in their course and personal schedule to complete 100 hours of clinical internship in 8 weeks during B block.

PLB 101 is the clinical portion of the course required for the ASCP phlebotomy certification exam and entails practice of phlebotomy skills at a clinical facility for a total of 100 hours.

- Graduation from the MLT program is not contingent upon passing a national certification examination. Students are not certified upon completion of the program. To become certified, graduates must apply for and successfully pass a national certification examination, such as the ASCP Board of Certification (BOC) exam. The Practical Nursing Program is a selective admission program determined by a points-based system (grades in Anatomy and Physiology, HESI score, and references). An incomplete file will not be considered. Complete the following by the application deadline:
- Complete OTC admissions process. Visit Admissions.

- a passing grade of “C” or better is not achieved after two attempts in any preadmission requirement, OR
- your anatomy and physiology sequence will be more than 5 years old at the time of your application deadline.
- Official transcripts from accredited schools attended other than OTC must be received by OTC Admissions before the application deadline.
- START the ONLINE APPLICATION PROCESS at apply.otc.edu. (Note: You will not be able to SUBMIT your application until you have taken your HESI Admission Assessment Exam.)
- Complete the HESI Admission Assessment Exam during your testing window. Upload results to complete and submit your application. (See testing windows below.)
- The TOEFL is required if English is not your primary language. (See additional information below.)
- If applying to the Advanced Hybrid Track (prior CNA/MA/EMT experience required), complete the work verification form HERE and upload it during the application process.
HESI ADMISSION ASSESSMENT EXAM
- A minimum overall score of 70% must be achieved to be considered.
- This test may be taken seated or online.
- Students will have 4 hours to complete the exam.
- Instructions are provided during the application process and on the Testing Services page.
- Testing cycles:
- February 1 – May 1
- July 1 – December 1
- You MUST take the exam in time to have results ready by the program deadline.
- You are allowed a total of 2 attempts per testing window.
- Cost: $75.00 online / $45.00 seated.

BEHAVIORAL ASSESSMENT
- Only students who complete the application process and meet the parameters by the cohort deadline are eligible.
- Eligible students will receive an invitation to a Canvas course called “NUR-Admissions.”
- Students will have one week to complete and submit the Behavioral Assessment.
- Students in the Advanced Hybrid cohort are exempt.
ENGLISH PROFICIENCY REQUIREMENT
- Students whose primary language is not English must document proficiency via TOEFL.
- Minimum TOEFL iBT score: 83 overall with 26 speaking.
- Submit current TOEFL iBT scores during the application process.
- Register through the TOEFL website — only professional test center exams accepted.
- Scores valid for 5 years from test date.
- This is separate from the OTC admission test.
NOTIFICATION OF ACCEPTANCE
- Top applicants based on the admission scoring system will be offered a position and given next steps for program admission.
- Notifications will be sent via the online application portal.
- Accepted students will receive an email with the acceptance packet task list.

A 2.75 GPA is required to be eligible for this program. An incomplete file will not be considered. Complete the following by the application deadline:

- a passing grade of “C” or better is not achieved after two attempts in any program preadmission requirement, OR
- your anatomy and physiology sequence will be more than 5 years old at the time of your application deadline.
- Official transcripts from accredited schools attended other than OTC must be received by OTC Admissions before the application deadline (preferably 4–6 weeks prior to allow time for review).
- Submit a Radiography application using the online application portal apply.otc.edu by the application deadline (preferably 4 weeks prior to the deadline).
- Complete a minimum of 4 hours of job shadowing prior to applying for the program. For more information about shadowing, access the Job Shadowing Experience Form, which should be uploaded during the online application process by the application deadline.
- Complete TOEFL testing if English is not the student’s native language and upload results during the online application process by the application deadline.
ENGLISH PROFICIENCY REQUIREMENT:
Students whose first language is not English must document their English language proficiency. This requires:
- Minimum TOEFL iBT passing score of 81, with a minimum speaking score of 24. *Program will waive TOEFL requirement if the student has already obtained a bachelor’s degree from a US regionally accredited University.*
- Student must satisfy this requirement by submitting a current TOEFL iBT score (Test of English as a Foreign Language) during the online application process by the application deadline.
- Testing is completed through the TOEFL website.
- The TOEFL report scores are valid for 5 years from the test date.
- This is separate from the test taken to be admitted to OTC.
SELECTION FOR INTERVIEW PROCESS:
Using the Candidate Selection Form, top-scoring applicants will receive an interview.
- All preadmission courses must be completed by the end of the semester in which the applicant is applying.
- Mid-term grades will be checked in the semester students are applying.
- A total GPA of 2.75 and higher, as well as a grade of C or higher in all pre-requisite coursework, is required to be considered a viable candidate.
INTERVIEW PROCESS:
- A message regarding the date, time, and location of the interview will be sent to your OTC email if you are selected.
- The interview is approximately one hour in length.
NOTIFICATION OF ACCEPTANCE:
- Based upon the program’s admission scoring system, the 18-24 applicants with the highest scores will be offered a position in the program and will receive information on the necessary steps to complete program admission.
- Information regarding acceptance or non-acceptance into the program will be delivered to students via the online application portal.
- The accepted students will receive an acceptance packet task list through their OTC email account to complete the admission process.

Points are awarded on the grades made in the BCS 165, BCS 205, BCS 200, BCS 210, and HSC 120; Bennett & Wonderlic score; math test; and references. - Provide 3 professional references as part of the online application process by the application deadline (April 3).
- Complete 4 or more hours of job shadowing for additional points by the application deadline (April 3). This is not required but recommended. Click here for the Job Shadowing form, which will be uploaded during the online application process.
- Complete Bennett & Wonderlic testing before the application deadline (April 3). Registration & information is found here. Upload all test results during the online application process. (Testing prior to spring break is highly recommended.)
TESTING:
- Students will receive scores upon completion of tests and must upload results during the online application process.
- Upload test results as instructed by the application portal and review carefully for accuracy before submitting.
- Students are allowed to take these tests once per application cycle.
- Payment is required for Bennett & Wonderlic tests.
- Scoring is outlined on the Candidate Selection form.
- To help you prepare for testing, the OTC Library has the following materials:
INTERVIEW PROCESS:
- Students receiving the highest level of admission points will receive an invitation to be interviewed.
- The Candidate Selection form outlines how points are accumulated.
- An email regarding the date, time, and location of the interview will be sent to the student’s OTC email account approximately 2 weeks in advance. Notify the Health Sciences Office of any change in name, email address, or phone number.
- Responsibility for failing to receive the interview notification due to incorrect contact information will fall to the student.
- Math testing is part of the interview process. A sample math test may be found here. Calculators will not be allowed.
- The interview will be approximately 15 minutes in length. The student is asked questions by a group of interviewers.
- Students may be interviewed pending successful completion of preadmission courses by the end of the summer. However, they will receive 0 points for these classes since they have not been completed.
