Request Proof of Enrollment or Attendance Verification

Get proof of your enrollment or authorize the release of your student information.

You may need an enrollment verification or release of information for reasons such as health insurance, student loan deferments, job applications, or housing requirements. These forms provide official confirmation that you are or were enrolled at Ozarks Tech.

Who should submit this request

Only students can submit these requests. Third parties such as employers, insurance providers, or background check agencies must request verification directly through the National Student Clearinghouse.

How to request a verification

Choose one of the following options:

• myOTC: Log in and select Enrollment Verifications to submit your request.
  
• National Student Clearinghouse: Visit Order-Track-Verify. Fees apply.
  
  • Degree Verification: $9.95
  • Verification of Dates of Attendance: $9.95
  • Verification of Current Enrollment: $2.50
    
• Verification of Enrollment and/or Request to Release Information Form: Complete this form through DocuSign.
  

If you request in-person pickup, your verification will be ready with a photo ID after 2:00 p.m. the next business day.
