CHAMPVA

You can get health coverage through CHAMPVA if you’re an eligible spouse or child of a qualifying Veteran. If you’re a dependent age 18–23, you must submit a school enrollment certification letter each term to verify your student status and keep your eligibility.

What you need:

• A school enrollment certification letter confirming your student status (required for dependents ages 18–23).
• The letter must be faxed directly from Ozarks Tech to CHAMPVA. Letters sent by students or parents are not accepted.

How to request the letter:

1. Submit the CHAMPVA request form for the relevant term to Ozarks Tech (see links below).
2. Ozarks Tech prepares your letter and faxes it directly to CHAMPVA.
3. Keep your contact information current in case follow-up is needed.
