Give another person or agency permission to access specific parts of your student record.
Under FERPA, Ozarks Tech cannot share your student information without your written consent. If you want to allow someone else ongoing access to certain records, you must submit the Release of Student Information Authorization Form. Multiple people or agencies can be listed on the same form.
This is not for sending transcripts to schools or employers.

Release of Student Information Authorization
When you complete the online consent form:
- You decide which records can be shared.
- Each recipient is required to provide a PIN before information is released.
- Released documents contain personally identifiable information and cannot be forwarded or shared with anyone else without your consent.
What the Release Allows (and Does Not Allow)
- Talk with staff in person, by phone, by email or by video chat about your records
- Request printed copies or official documents
- The consent form does not give access to your myOTC portal, Canvas, email, or any secure system.
- Records are only provided upon request. Information is not automatically sent.
- FERPA only permits sharing records. It does not give anyone the right to act on your behalf or make changes to your record.
Timing and Duration of Access
- Requests for records may take 1–2 weeks to process, depending on the type of record.
- Written consent lasts for one year from the start date, unless you set an earlier end date.
- You may revoke access at any time by updating the end date on your form in myOTC.
- If no end date is provided, access automatically ends after one year.
Conditions and Limitations
- The Registrar’s Office may contact you to confirm that you completed the form before releasing records.
- The Registrar’s Office may decline to accept a form if it appears you are completing it under duress or coercion.
- This form is not required for Ozarks Tech to comply with lawful subpoenas or judicial orders, including ex parte orders under the USA Patriot Act
Note on Proxy Access
If you want someone to be able to log in and view online the parts of your student record you designate through their own myOTC account, set up Proxy access instead. With Proxy access the person you grant access will be able to see certain information but cannot talk with staff about that information.
